FARGO ALL CITY YOUTH HOCKEY 2004/5 PROGRAM REGISTRATION: Registration for termites, mites, 8U, 10U, 12U, and 16U Girls in person or by mail.

Required Items for Registration: Player’s Birth Certificate (copy)
*Complimentary Snacks will be provided by: Hornbacher’s Foods & Club’s Choice
*Enjoy free skating - Bring your skates or borrow a pair provided by Play It Again Sports.
**Meet the FM JETS Junior A Hockey Team and Skate with the Players! Register by mail:
Deadline to mail registration is September 1, 2004. The Registration form, Waiver of Liability, Consent to Treat and Medical History forms all must be completed and Mailed by September 1st.
Programs Ages Season Termites (co-ed): Born on or after 7/1/97 November 4 – March 6 Mites City, Mite Travel & 8U Girls Born between 7/1/95 and 6/30/97 October 3 – March 13 10U Girls: Born between 7/1/93 and 6/30/95 October 3 – March 13 12U Girls: Born between 7/1/91 and 6/30/93 October 3 – March 13 16U Girls: Born between 7/1/87 and 6/30/91 October 3 – March 13
